The Health Visiting Team

The Health Visiting Team at the Practice comprises of Oona Emerson and Sue Freeman (Health Visitors), Tamasin Warrell (Community Nursery Nurse) and Claudette Newman (Team Administrator). The team are based at the practice on a part time basis however messages may be left on their answer phone or at the surgery at any time and calls will be returned as soon as possible. The Health Visitors have a dedicated telephone number which is 020 7627 3341.

Patients may expect to see a Health Visitor approximately 10 days after giving birth when a ‘red book’ should be given to the new mother. This is used to record all child health consultations and information such as weight and immunisations. This book should be brought along to all child health or health visitor appointments.

The team work with all families whom have young children and promote child and family health. Families may be seen at home, by appointment at the surgery or during the well child clinic on Wednesdays between 1.30pm and 3.30pm. The team work closely with other agencies such as Social Services to safeguard vulnerable children.

 

The Attached Staff

Jonathan Smith

Jonathan Smith is a qualified Psychotherapist registered with the UKCP. He is employed by the South West London & St Georges Mental Health Trust and works at the practice part time.

Jonathan offers a counselling service to patients who are experiencing difficulties such as depression, anxiety, relationship problems, bereavement, panic attacks, phobias or a difficult emotional response to a physical illness. He will usually offer one or two sessions in which both he and the patient will together make some assessment of the problem and how counselling could assist in resolving it. Patient may then be offered a further twelve sessions and a follow up session. Jonathan Smith’s timetable is as follows and is subject to change:

Tuesday 12pm – 6.30pm and Wednesday 9am – 12.30pm/ 4pm – 6.30pm

 

 

Physiotherapist

The physiotherapist works at the practice part time one morning and one afternoon per week. She treats patients with a range of musculoskeletal conditions. Patients are referred by the GP. There is a waiting list and depending on the severity of the condition patients may be seen from one to within 8 weeks of referral.

Dietician

The Community Dietician, employed by Wandsworth PCT and based at the practice part time and runs two sessions per month on a Monday afternoon and Wednesday morning. Patients must be referred by a doctor or practice nurse to Sharleen who sees patients with a range of conditions including diabetes, heart disease, obesity and irritable bowel syndrome. Advice is given on all aspects of diet and nutrition.
