Medical Examinations, Forms & Certificates
The Doctors may receive medical report requests from various outside organisations for various reasons. If the medical request requires an examination, a 30-minute appointment should be made. These examinations are mostly chargeable as they are not part of NHS services . No information will be given to a third party without signed authorisation.
Other forms, for example, Orange Badge forms should be completed, where applicable, fully before being handed into the doctor for completion of the medical form. Failure to fully complete any form will result in it being rejected.
Certificates for employers etc should be requested at the point of consultation where possible. Unfortunately doctors are unable to complete a certificate if you have not actually been seen with the condition for which you require it. Certificates for National Insurance purposes are completed free of charge; private certificates are chargeable at £10. Statutory Sick Pay forms (SC2) are available from reception if your absence from work has been for less than 7 working days. See below for a full list of charges.
